BAD WEATHER

In bad weather please listen to WLBK (1360 AM), WNIJ (89.5 FM), or WDKB (94.9 FM) to find out whether CLC will be open. In most cases an announcement will be on the radio by 6:00 a.m. Occasionally we are forced to close during the day; in this case we will notify parents.

When DeKalb Schools are closed, CLC's Lincoln and Littlejohn programs will not operate, although the day care center may remain open.

FORMS

• Child Information Update: Use this form to make changes to parent/guardian contact information, parent/guardian work/school schedule, and general health information.
• Add/Remove Pickup Person: CLC will release your child only to persons who are at least fourteen years old, have written authorization from a parent or guardian, AND have photo identification. NOTE: Any indication that either parent may not pick up a child requires official documentation to be provided to CLC.
• Medication Form: CLC will not administer the first dose of any medication or fever reducers.
• Vacation Notice Form / Vacacione Forma de Nota: CLC will hold a space for up to 2 weeks for vacations.

SCHOOL-AGE HOLIDAY PROGRAM SCHEDULES / LOCATIONS

• 2011-2012 School-age Program--Holiday Dates and Locations: For CLC families with children in kindergarten through fifth grade, CLC offers full- and half-day care on many days when school is not in session, pending available space.
