Thank you for your interest in enrolling your child in Children's Learning Center (CLC)! If you would like to enroll your child, please come visit our center and complete a Child Care Interest form (English) or El Cuestionario de la Guardería está también disponible (Español). You must have Adobe Acrobat Reader to download this form. You may also download the form and bring it to the Main Center, or you may send it via mail or fax.
Children are enrolled as space is available, according to priority, considering each family's household status, if a child has enrolled siblings, if a child has been enrolled previously, and if a family receives financial assistance, among others. Open spaces are filled first from the waiting list, if applicable, and then on a first-come first-served basis.
ENROLLMENT & ORIENTATION
Once a space has been offered to you, a non-refundable $40.00 new family registration fee is due to secure the space. This payment must be made at the time you pick up the CLC paperwork packet (if you have not already downloaded and completed the paperwork at home--see list below). You will then schedule two (2) appointments, both of which are necessary to complete the Enrollment and Orientation process:
FIRST VISIT: For Parent/Guardian (only, if possible): This 30-60 minute meeting helps you become acquainted with the CLC administrative staff. You will also receive and complete required paperwork, if you have not done so already.
SECOND VISIT: For Parent/Guardian & Child - This 20-30 minute visit includes meeting your child's teachers, exploring the classroom, and getting to know each other. At this time, any outstanding paperwork must be submitted.
In general, a child can begin attending two (2) business days after his/her paperwork file completed. This ensures that all necessary information is complete, filed, and entered into our system. During this period, your child's teachers will also prepare their classroom for your child. To expedite enrollment, please bring with you the items on the list below; the more complete your paperwork is, the sooner your child may begin.
PAPERWORK LIST
This list is intended for families that have already been offered a space in a CLC program. Do not complete these forms until you have confirmed with CLC that space is available.
1. Items you may already have at home (bring completed to first visit, if possible):
- Certified Birth Certificate - This document must be stamped with the official seal. If your child was born in DeKalb County, you may obtain an official birth certificate at the DeKalb County Clerk’s office.
- Certificate of Child Health Exam (a/k/a "Physical" form) - For infants, toddlers, and preschool children, this form must be signed by a physician and dated within six (6) months of enrollment. For children in kindergarten through fifth grade, the most recent elementary school exam form will suffice. In either case, the following also must be completed :
- Up-to-date immunizations,
- Lead risk assessment or test date and result (ages 6 weeks to 6 years only), and
- TB risk assessment or test date and result.
- 4-C Child Care Subsidy Packet (if applicable) - Please contact 4-C for eligibility requirements and necessary documentation. You may also obtain some forms in advance from the 4-C website.
